Our caring team of professionals at CHP Home Care & Hospice is dedicated to delivering exceptional care to every patient. Our care team of local, skilled professionals work together to assist patients and families. The CHP staff is dedicated to the safety and well-being of all our patients. This collective team strives for shared goals within and across disciplines and settings to achieve coordinated, high-quality care.

CHP Home Care & Hospice Team

MEDICAL DIRECTOR

The CHP Home Care medical director approves all medical policies and can review and sign a patient’s plan of care in the absence of the patient’s primary care physician. The CHP Hospice medical director offers expertise in pain and symptom management and bedside visits to the patient as needed. The medical director collaborates with the hospice team and the patient’s physician. The medical director, in collaboration with the patient’s physician, certifies that the patient is appropriate for hospice care and gives approval for recertification.

NURSING SUPERVISOR

Each CHP Home Care & Hospice office is overseen by a nursing supervisor. The nursing supervisor works directly with the director of nursing, VP of clinical services, and medical directors to ensure that care is provided according to each patient’s plan of care. The nursing supervisor is responsible for the coordination of patient services and is available to provide evaluations for palliative and hospice care. The nursing supervisor also provides consultation to patients and families.

HOME CARE & HOSPICE NURSES

Nurses from CHP Home Care & Hospice provide loving, hands-on care to patients in their homes – wherever they call home. These caring providers are trained to deliver general nursing care and specialized procedures. CHP nurses are also cross-trained to provide palliative and hospice care. Their expertise in pain and symptom management helps keep patient’s comfortable. They coordinate services and manage the patient’s plan of care. Hospice nurses collaborate with other team members, the patient’s physician, and facility staff to ensure the patient’s needs are being met. A CHP Hospice nurse is available 24 hours a day, seven days a week. CHP nurses provide education to families and facility staff.

HOME HEALTH AIDES

Provide one-on-one supportive and personal care for the patient. Personal care includes hair care, nail care, bathing, mouth care, range of motion, and vital sign monitoring. CHP Home Care aides may also provide companionship to the patient and assistance with day-to-day activities, including light housekeeping tasks.

SOCIAL WORKER

The CHP Home Care & Hospice medical social worker provides emotional support and assist to patients and family. The social worker facilitates positive coping with adjustment to illness and bereavement needs. They also offer assistance with financial concerns and community and caregiving resources. Our social work team educates patients and families on hospice, advance directives, living wills, and end-of-life issues.

CHAPLAINS

CHP offers non-denominational spiritual support to hospice patients and families. The chaplain is available to assess and address spiritual/pastoral, bereavement, and general counseling needs. CHP chaplains are trained ministers, psychologists, or counselors. They work effectively with people of all faiths, beliefs, philosophies, and values.

BEREAVEMENT MUSICIAN

CHP offers non-denominational spiritual support to hospice patients and families. The chaplain is available to assess and address spiritual/pastoral, bereavement, and general counseling needs. CHP chaplains are trained ministers, psychologists, or counselors. They work effectively with people of all faiths, beliefs, philosophies, and values.

VOLUNTEERS

Under the direction of our volunteer coordinator, CHP Hospice volunteers are trained to help provide companionship, socialization, assistance with errands, reading with a patient, veteran recognition, and more. In addition, volunteers provide administrative and fundraising support for CHP Hospice.
