Accessing Care

Many of our services are initiated through a referral from your doctor, hospital, or other health care providers. However, you have the right to choose who you want to provide your home health and hospice services. Be sure to ASK FOR CHP! Otherwise, you may be referred to another provider that's not locally-based.

Who can make a referral?

A Patient
Family Members
Healthcare Providers and Physicians

A referral from outside the medical community starts with a free, no obligation consultation with one of our nurses. The consultation can be in person or by phone.

Our trained admission nurses help guide patients and families and answer any questions.

A nurse can meet with a patient and/or family members in the home, a nursing facility, or hospital, for an admission evaluation. Once complete, our team will contact the patient’s primary care physician to determine admission criteria for any of our services.

Admission Policy

In order to accept a patient for services, CHP will evaluate:

Anticipated needs of the referred prospective patient
Our current caseload and case mix
Our current staffing levels
Skills and competencies of our staff to meet the patient’s needs
Insurance coverage to assure contracted services

PRIVATE INSURANCE

INPATIENT HOSPICE ROOM & BOARD FEES

Costs associated with living at the CHP Inpatient Hospice Center are not covered by Medicare, or by most private insurance plans. Residential care is needed when a patient’s medical condition is stable, but the patient is no longer able or willing to remain at home. Room and board fees are billed on a sliding fee scale. Our social workers can assist patients and their families to secure financial assistance when needed.

INABILITY TO PAY

As a nonprofit organization, we strive to work with all patients and families, regardless of financial status. The first thing CHP Hospice will do is assist families in finding out whether the patient is eligible for any coverage they may not be aware of. Barring this, our patient care fund – which is funded through donations and community fundraising events – helps to provide for uncovered costs.
