Starting Services

Initiating services with CHP starts with a call to 419-238-9223. Our friendly staff will work out the details to get things setup.

Who can make a referral?

• Healthcare Providers and Physicians

A referral from outside the medical community starts with a free, no obligation consultation with one of our nurses. The consultation can be in person or by phone.

Our trained admission nurses help guide patients and families and answer any questions.

A nurse can meet with a patient and/or family members in the home, a nursing facility, or hospital, for an admission evaluation. Once complete, our team will contact the patient’s primary care physician to determine admission criteria for skilled home health care, hospice, or Palliative Care.
