How to start a referral

Making a referral to start services with CHP is easy…just give us a call! Our staff will discuss your situation and suggest which service is right for you. We’ll work out the details with your physician to get things setup.

 

Who can make a referral?

A referral can be made by:

  • A Patient
  • Family Members
  • Healthcare Providers and Physicians

 

A referral from outside the medical community starts with a free, no obligation consultation with one of our caring nurses. The consultation can be in person or via phone call. Our trained admission nurses will help guide patients and families and answer any questions you may have. 

A nurse will meet with the patient and family wherever they call home, whether that be the patient’s home, a nursing facility, or hospital, to evaluate the patient’s current condition and review medical records. Once the evaluation is complete, our team will collaborate with the patient’s primary care doctor to determine if the patient meets the criteria for admission to skilled home health care or hospice.

If the patient would not meet the criteria for starting hospice services, we offer Palliative Care services through our skilled home health care program.
