Why give to Hospice?

By giving to CHP Hospice, you honor the memory of your loved one and help ensure that our services will be available to other families in their time of need. What a wonderful, lasting way to carry on the memory of those we love.

Hospice is covered by Medicare, Medicaid, and most insurances. However, a patient may have to quit their job due to illness and lose their insurance. Patients may have to wait for authorization, but their needs can’t wait. The CHP Patient Care Fund enables us to deliver timely care to ALL hospice patients. 

CHP Patient Care Fund in Action:

  • Brad, a 40-year-old with cancer, had to quit his job, and lost his insurance – he received hospice services from CHP
  • Mary needed a medication for pain control, but insurance had not yet authorized it. She received the necessary meds through the patient care fund and was kept comfortable.

CHP is a nonprofit, 501c3 organization. We strive to be efficient with our funding and work to make the most of every dollar. Your donations and support of our events enable us to continue providing compassionate care to those in our community facing the end of life. 

Direct Your Gift

On the donation form, you will designate which CHP office and hospice program you wish to support :

  • In-Home Hospice – Patient Care

  • Inpatient Hospice – Patient Care

 

Memorial Donation Acknowledgement

You also have the option to make your donation in memory of a specific patient, and to have an acknowledgement sent to the family of the patient you listed.

 

Payments are made securely through PayPal, using either your account, or any major credit card.

 

 

 

You will receive email notification and a written receipt after we receive confirmation of your donation from PayPal. In most cases, you will receive email notification within 1-2 business days.
