TeleHealth

As hospitals discharge their patients earlier, the need for recovery care at home becomes more evident. By utilizing HomMed® TeleHealth, Community Health Professionals is able to bring advanced equipment to you, wherever you call home, allowing us to detect irregularities or warning signs of health complications earlier.

This gives you peace of mind, knowing you can depend on us to be by your side 24/7. Daily monitoring also helps your physician be more aware and pro-active in your care, allowing for day-to-day adjustments to medications or the treatment plan.


What is HomMed® TeleHealth?


A HomMed® TeleHealth system is kept in your residence to collect vital information and clinical data regarding your condition on a daily basis. This data is transmitted via a standard telephone connection or other wireless/digital network to our central office, where nurses monitor and review the data being updated.

If the data shows unexpected readings, the clinician will contact you with instructions to re-monitor, contact your physician, send a nurse to assist you or contact emergency personnel directly, depending on the severity of the issue.


Who can it help?


Patients who have been discharged from a care facility (e.g. hospital) or diagnosed with chronic or co-occurring conditions, such as congestive heart failure, diabetes or high-blood pressure, are among those we recommend be placed on a HomMed® TeleHealth in-home monitoring system.


Paying for HomMed® Telehealth


We evaluate you for the appropriateness of TeleHealth monitoring and monitor you at no additional cost, if it is prescribed by a physician. Other patients who are not eligible for skilled home health care can still request to be monitored for a nominal fee.
